Diabetes Community Adult Specialist Nursing Service

The service provides diabetes specialist advice, guidance and support to adults with diabetes, their carers and health care professionals within Southampton City.

Referral Criteria

Adults registered with a Southampton City GP.
 
Most adult diabetes care is managed within in GP surgeries. However, if specialist support is required a healthcare professional referral is required. This maybe for group education, an appointment within the GP surgery to see the diabetes specialist nurse or to be seen in the Diabetes Centre by one or all of the multidisciplinary team (Consultant, diabetes specialist nurse, dietitian)

Full service description

The service is Consultant supported and led by a team of diabetes specialist nurses. The aim is to provide diabetes specialist advice and guidance by supporting and working alongside health care professionals who are looking after people with diabetes in the Community and within GP practices. 
In general the first point of contact for people with diabetes within Southampton City should be their GP practice team.  The practice team can then arrange for you to been seen by the most appropriate person. This maybe in the form of a joint appointment with your practice nurse or GP and the community diabetes specialist nurse. This arrangement enables the person with diabetes to be seen by the people that know them whilst having access to specialist input, close to home, when needed. 
 
For those people with diabetes already referred to the service by their health care professional there is access to advice via email or for  a more urgent concern an urgent call line for same day response.  There is also an opportunity for face to face appointments with the diabetes specialist nurse, dietitian and if required, the diabetes consultant. Consultant clinics run twice per week from the Diabetes Centre, Royal South Hants Hospital. 
    
The service delivers a range of group education sessions for those with diabetes - please ask your GP or practice nurse regarding a referral to the appropriate group.
